To connect the customer’s facility to the GTS it is necessary:

1. Fill in, sign and send the original Application for connection of the customer’s facility to the GTS (6.1 UA.docx);
2. Fill in, sign and send the Questionnaire of technical parameters of the customer’s facility (6.2 UA.docx);
3. Copies of documents confirming the existence of ownership or use rights in a connection customer to the facility planned for connection, or to the land plot where it is planned to build such a facility;
4. Copies of the customer’s documents confirming the authority of the representative for the right to enter into a connection agreement;
5. Draft act and scheme for delimitation of balance ownership and operational responsibility of the parties.

The list of documents confirming the authority of the representative for the right to enter into a connection agreement:

• A copy of the charter of the counterparty, which is certified by an authorized person of the enterprise;
• A copy of the order on the appointment of director-general/chair of the board of the counterparty, which is certified by an authorized person of the enterprise;
• A copy of the minutes of the shareholders’ meeting on the appointment of the director-general/chair of the board of the counterparty, which is certified by an authorized person of the enterprise;
• Certified copy of documents for signature verification;
• A copy of the Power of Attorney of the signatory, network user.
